How responsibilities are divided:Derby, Derbyshire, Nottingham and Nottinghamshire are covered by the East Midlands Combined County Authority for selected devolved functions. Greater Lincolnshire has its own combined county authority. SEND and EHCP duties remain with the responsible council, while district councils may hold separate records for housing, local planning and environmental services.
Boundary and transition note:North Lincolnshire is commonly classified within Yorkshire and the Humber, but it is included here exactly as requested and also participates in Greater Lincolnshire arrangements. Greater Lincolnshire local-government reorganisation is developing, so verify the responsible authority when consulting future or transitional records.
Independent disclaimer:Right First Time UK is not operated by or affiliated with any council, combined authority, NHS body, police force, fire service or government department. Official websites and processes can change. Verify forms, deadlines, policies and contacts with the responsible body.

Public transparency checklist

  1. Identify whether the council, district council, combined county authority, NHS body, police force or another organisation owns the decision.
  2. Search committee papers, forward plans, delegated decisions and registers before submitting an FOI request.
  3. Save the report, meeting date, agenda item, decision-maker, vote and declared interests.
  4. Compare the approved budget or business case with monitoring reports, final spending and measured outcomes.
  5. Search council contract registers, Contracts Finder and Find a Tender.
  6. For planning, retain the officer report, decision notice, conditions, obligations and appeal records.
  7. Use FOI for public information, EIR for environmental information and SAR for your own personal data.
  8. Request an internal review where an FOI or EIR response appears incomplete or poorly reasoned.
  9. Complete the relevant complaints process before approaching an Ombudsman unless a statutory appeal applies.
  10. Retain dated copies because websites and authority structures can change.

EHCP accountability checklist

  1. Save proof of when the council received the assessment request.
  2. Track the six-week decision and overall statutory process.
  3. Request every education, health, psychology and social-care advice document.
  4. Ensure Section B records all needs and Section F specifies and quantifies provision.
  5. Check Sections G and H for relevant health and social-care provision.
  6. Respond in writing to the draft plan and state the preferred placement.
  7. Record every occasion when final-plan provision is not delivered.
  8. Use annual reviews to update needs, outcomes, provision and placement.
  9. Use tribunal rights for appealable decisions and complaints for delay, maladministration or service failure.
